Stanmore Medical Group seek a keen and enthusiastic General Practitioner (GP) for Care Homes to join our progressive, dynamic, technologically driven, expanding and modern training practice.

6 to 8 Sessions – flexible on hours

We believe that the most vulnerable and frail patients at home and in care homes deserve the best care we can offer. We are therefore looking for a GP with a passion for elderly/palliative care to help our talented teams support these patients.

Are you a GP (2 years+ CCT) with experience of elderly care and palliative care and interested in providing support for patients in Care Homes/frail patients in the Community?

GP Care Home Lead:

• To provide clinical leadership to our complex care clinical team delivering services to
residential care homes

• Provide medical assessment and management where needed, or in conjunction with, other team members to deliver the care home visiting service such as urgency and type of response needed according to clinical need

• Arrange and chair regular debrief sessions team meetings to manage patient case load, advising on complex clinical situations

• Promoting continuous improvement by leading and delivering responsive, safe, patient centred and effective care and support to elderly, frail and palliative care patients

• Supporting frailty projects in the community for Stanmore Medical Group patients

• Running our PCN MDTs for Care Home patients

The Practice has a friendly and loyal complement of staff consisting of 15 Partners, 9 Salaried GPs, 3 Advanced Nurse Practitioners, 5 Emergency Care Practitioners, 16 Nurses and 5 HCAs along with an experienced and supportive managerial and administrative team.

We are based over several sites within Stevenage, Hertfordshire. We are committed to delivering a high quality service to our 50,000 patients. Each GP has a personal list to facilitate continuity of care and low home visit demand. The Practice is a high achieving QOF practice working on SystmOne with an overall “Good” CQC rating.

We are the Lead Practice within the well-established PCN and supported by Clinical Pharmacists, Pharmacy Technicians, Social Prescribers and Care Co-ordinators. We work closely with other practices in the locality.

We believe in working hard and supporting each other as a team, to achieve a sensible work/life balance, whilst providing good patient care.

We are a Training Practice offer training for foundation year doctors and GP registrars.

We offer excellent career opportunities for the right candidate to join our hard working team. Salary of £11,000 to £11,500 per session is dependent on experience – BMA Model Contract.
