PCNs

Primary Care Networks (PCNs) are groups of general practices working closely together with other primary and community care staff and health and care organisations, providing integrated services to their local populations.

NHSE describe the development of PCNs with the aim to “build on existing primary care services and enable greater provision of proactive, personalised, coordinated and more integrated health and social care for people close to home”.

PCNs are led by clinical directors who may be a GP, general practice nurse, clinical pharmacist or other clinical profession working in general practice.
